*Game Report Form Procedure:
League rules require the head referee to :

1. have the game report form filled at the game
2. get both teams' signed line-up sheets
3. mail both teams' line-up sheets along with the game report form to:

Home Team is responsible to report the game result by Monday noon using one of the following methods:

• electronically through the League's website (go to Teams Report Results ) or
• by e-mail to webmaster@lvotsl.org or
• by calling 610-390-4998
